When I was in the Lost Refuge and opened that last chest, then two windows appeared on top of each other: The dialog window and the quest reward window. This is entirely normal. However, clicking A closed the dialog window, which was (partially) hidden by the quest reward window, and clicking again did not close the quest reward window, even though the button was highlighted. I needed to use the (right thumb stick) cursor to do so.
