I have a quest to accept a prophecy (in which I do, and it will act as if I have accepted it), and then I wander off and it acts as if I haven’t just accepted it, despite saying ‘Yes’ to the prompt to accept it.
The only beauty is that it doesn’t spend my favor, but it keeps re-rolling for free pretty much as the prophecy that I ‘accepted’ is no longer there.
Pls fix!

I have the same problem. Purchase prophecy, agree to reroll requirement, close menu but no prophecy purchased. Open up the prophecy page again and new prophecies. Can’t complete the quest or get any prophecies.
i have the same problem. it acts like it works but then it rerolls and refunds me.
google led me to believe it’d fix itself if i gain more reputation levels with my faction.

I don’t know if the new constellation is supposed to appear right after the confirmation, but the lens stays empty. The reroll is only visible after leaving the telescope and getting back to it.
Another detail, the cost of the prophecy is substracted from the favor on top, but if you move the telescope to see another constellation, the game considers you still have your previous amount.
… aaaand while I was testing, it seems it is fixed… The constellation is now rerolled instantly and the quest is completed. Go figure ! 
Update, works fine now it seems. Not sure what the deal was yesterday but all works as intended as of today.
I am still having this same issue even after the update…
i came back once level 4 with the faction and it started working.

Except it’s eating my favor and losing out on good prophecy, I lost a bunch and wasted a ton for favor before I realized what was going on.
I have no explanation other than that I just walked away from this system for a few days, ranked up naturally, a couple of minor patches dropped, and now it works fine for me.
I just ran into this bug.
First I purchased a prophecy and the constellation went away and was not replaced. Exited the telescope and checked prophecies. No prophecy and no favor spent.
Checked telescope and there was a different constellation now.
Went to another constellation to find a prophecy I could afford. Purchased it, and noticed that more favor was spent than I expected. Checked prophecies and it turns out I received a different one than I had purchased - one I did not want at all.
I have hit the same bug today with my online character. In the screen the favors go down, and the prophecy rerolls, but as soon as you walk away from the telescope it returns your favors and no prophecy is activated. I have tried leaving and teleporting back in, and also trying to activate prophecies from different telescopes, but all have the same behaviour.

Still happening weeks after it was reported. This really halts progression and should be fixed within a day or two, not weeks or months!
Same thing is happening to me today. I’ve only had 1 single prophecy “Succeed” when I accept it. Every other prophecy I’ve accepted disappears and never gets added to my “Prophecies” list. The constellation screen clears after I accept, then I esc out, and go right back in and my Constellation has reset, but no prophecy gets added at all. It does appear that my favor does not get spent, but its really frustrating having this happen. One single prophecy I’ve been able to perform. One.
I did send a bug report in game, but since it appears to be an ongoing issue, who knows if it’ll get fix anytime soon.
