While playing the campaign with my level 27 Lich and a legendary dropped. However, it fell out of bounds and I’m unable to pick it up. 
I have pictures but seem unable to upload them here (Posted in Discord feedback channel). very frustrating to finally get some good drops and not be able to pickup. Total kill joy.

Seems this has happened to multiple people (for me too).
There are some barrels hanging in the air in Imperial Welryn
Similar issue in Imperial Majasan Desert. Dropped while pets were fighting off screen, so I’m not sure if it dropped from a mob or a barrel that was out of place like Maximi described above, but this one got stuck in a rock.
