I died against the boss in Ash Covered Maj’elka and any attempt to re-enter the zone comes without any of the objective triggers. I burned about 16 echoes trying to re-enter and the objectives never trigger again. I even re-killed the pyres without the objective and still couldn’t unlock the flame barrier for the boss.
This is a known issue, we’re tracking this internally. Out of curiosity, did the boss happen to die after you did?
