My understanding is that a rare item can have a maximum of 2 suffixes. While inspecting my items I noticed an amulet having 1 prefix and 3 suffixes. https://imgur.com/fXOCMHm

That’ll be a legacy item from prior to 0.8.1 when they reworked some of the affixes.
