When I maxed added dodge rating it maxed out both increase/added on my belt even though the prefix was tier 3.

Yeah, that looks like it’s a display issue on the belt prefix since it’s not removed the dodge prefix from the list of craftable affixes as it has with the t5 suffix.
Yup, could be because they both have the same name even though being two different affixes. I noticed once I updated the increased dodge rating it wasn’t red anymore. Hella confusing when it happened though.
