I had some items (helmets) with cooldown reduction on it.
Since 0.9 the cooldown reduction affix changed from prefix to suffix. this caused those items to have 3 suffixes and 1 prefix with one of the suffixes being unable to be changed since in the forge window only 2 suffixes are visible.
Were these old patch version items? If they were then this is a common problem with old gear that doesnt convert in newer patches. Usually when affixes have moved around but in other situations too. There isnt anything to do but drop or shatter the items. While some of them tend to work ok, I wouldnt recommend using them as they may bork something going forward - I have had a few on previous patches (not 0.9) that corrupted things.
