What happened:
I changed zone through a portal while still having an item on the cursor (the staff I have equipped in the video). After the new zone loaded I equipped that item (staff) which was still on my cursor. After doing so the icon of the new staff was stuck to the replaced staff. This persisted when moving the old staff around or dropping it as can be seen in the video.
Relogging fixed the issue.
I do not have a logfile available currently.
