What were you doing at the time?
Mapping in Black Sun, and using Lunge in the general direction of a monster. The auto-targeting chooses a breakable that was generated in a small notch of the map, locking me within. Enemy range attacks can still hit me, and I could barely move. Teleporting was possible.
How consistently does this happen?
This was the only time it happened.
