I am trying to complete the quest Sanity in the Darkness. I am missing the last Phylactery, but it isn’t there. I know I finished it but it is telling me I still need the last Phylactery. I can’t turn it in or progress. I did find a posting that someone else had also had this issue but I don’t see that it was ever resolved or an explanation of what to do to fix it.
Have you tried going back to town and restarting the game? It should reset the quest state to a point where you can do it.
