In the Corrupted Lake Ruined Era side quest I noticed after killing the 2 bosses, Prophet of Ruin and Idol of Ruin there was an add, a void leech, from the Idol of Ruin remaining and attacking me. I am unable to target or damage this mob, even with a non-melee attack. I walked back to the rift at the beginning of the area and the void leech followed me all the way across the map to the rift. I used the rift to go back to the Imperial Era and returned to the Ruined Era and the void leech disappeared. I went back and re-killed the boss and it did not happen again.
I ran into this problem as well once out of 300 kills or so. Very hard to reproduce. If you figure out what happened to cause the worm to become untargetable report back.
