I’ve had an item drop with a broken nameplate. I disabled the loot filter to see if any other items on the ground could cause it, but none had “Ursine” in the name. Also, disabling the loot filter fixed the issue.

Just had this happen again and noticed something: The name shown on the broken item nameplate (a unique item, again) was the first word of the last item I had picked up, which was also emphasized (which isn’t working for me) and recolored by my item filter. There may be a connection there. Just thought it might help in case this isn’t fixed already anyway. 
This occured to me today for the second time since starting to play LE. I’ll post a screenshot here.
When I repeatedly hovered the item with the mouse pointer the name pl ate corrected itself, also the item was ok in the inventory. Only messed up on the ground.
I had this happen to me as well, not with a unique item. I didn’t do any extensive poking around, I didn’t even pick up the item, but I thought I’d share the screenshot I took.
