Bosses teleporting out of bounds

This happened to me on 2 occasions - one was in an Echo where the boss teleported inside the wall - couldn’t see him but could still attack him. Once dead the loot was out of reach.

The second occurrence was early in campaign (the big shade boss) who teleported just outside of the arena and was floating in the air. He kept saying his voice lines but no abilities or movements were done. The loot dropped partly out of bounds, partly on the edge of the arena.

I have screenshots for both and happy to send them on DM if required.

Pannion teleported himself into a wall in a lvl 90 monolith

Not sure what I can add really. The majority of the fight was normal, until he teleported right next to a wall, his next teleport was then to the inside of the cliff. I suspect he’s teleporting directly away from the character & the game doesn’t check whether his intended target location is valid. [source: imgur.com]
