Ballistae don't hit mobs on a lower level than them

As you can see, the Ballistae fire their projectiles horizontally, not towards the target.

Yep, it’s pretty common, even slightest terrain changes can cause ballista’e miss, i thought it’s intended

I would assume not, Disintegrate has the same issue. I think the only reason why Thorn Totem/Ice Thorns hit mobs is that they track their target.
