I noticed that there is a visual disconnect on the character selection screen when a character is holding Arek’s Bones. You can see in the pic here that the item is actually floating below the hand that is supposed to be holding it.

This doesn’t happen with other items I tried so it appears to be this specific item that is the issue. Two different characters of different classes to show it isn’t the class.
