Ability bar clearing out on Primalist

Sporadically when playing on a Primalist, my ability bar clears all abilities leaving me with just move on left click and Ice Thorns on right click. This issue has occurred several times over the last two days. I can’t determine the exact cause but it seems to occur sometimes after character selection, zone transition, and teleporting to other players. I saw a similar issue has affected Spriggan Form in the past, which I am using. I can also confirm that none of my friends playing Primalist, not using Spriggan Form, have experienced this issue nor have I ran into it on my Sentinel.

Adding my last Player.log file were this issue occurred about six times in two hours. Trying to recreate the circumstances of each wipe after it occurred would not wipe the abilities again.

Same here, lost my ability bar after zone transition. It happen very often, I would say something like 1 time of 5.
